The following are Prophylactic Regimens for the prevention of Bacterial Endocarditis:

Dental, Oral, Respiratory or Esophageal Procedures:

Regimen 50 mg/kg/dose 1 hour before procedure 20 mg/kg 1 hour before procedure

Agent Cephalexin or Cefadroxil Azithromycin or Clarithromycin

Agent Clindamycin Cefazolin

Vancomycin 20 mg/kg IV & Gentamicin 1.5 mg/kg IM/IV
